Click here for the 2008 Food Establishment Permit application. Please read the following instructions to avoid any delay in receiving your permit. Operating without a permit in Warren County is a prosecutable offense (see below).

Do not confuse this fee with your Merchant' s License fee!
Use your 2007 gross retail sales (food and beverages) to determine your fee.
The form and fee are due by January 1, 2008. If payment comes from your corporate office, it is the responsibility of the manager to forward this request to your corporate office for timely payment. Establishments that have not paid by this date are in violation of the Warren County Ordinance that governs food establishments and could have their business closed and/or prosecuted by the Warren County Prosecuting Attorney.
The permit will be mailed to the establishment when payment is received.
The current food establishment permit must be placed in public view.

Food Service Inspections:

The Warren County Health Department, Office of Environmental Public Health conducts food service inspections under the authority of Warren County Ordinance No. 96-02. This Ordinance sets forth standards by which sources of food, food service establishments, food service personnel, and food service equipment are regulated to protect the health, safety, and welfare of the public. Critical items relate directly to factors which lead to Foodborne illness, these items must receive immediate action. Non-critical items relate to the maintenance of food operations and cleanliness, these items are to be corrected by the next regular inspection.

Comments: This re-inspection was to verify a prior critical violation has been corrected, non-critical items will be reviewed during your next routine inspection. Critical Violations: Ice cream mix and many other products in walk-in-cooler not at proper temperatures, product was destroyed. Non-Critical Violations: Ventilation hood not functional. Comments: This complaint inspection due to a complaint of ventilation of hood not working, bare hand contact with ready-to-eat foods, back door open, flies, and cooler not keeping food cold. Only items related to this complaint were reviewed. The back door was not open, only a few flies present (pest control is in place), no bare hand contact with ready-to-eat food was observed.
