Birth and Death Certificates

Warren County Health Department provides certified copies of Birth and Death Certificates. This service is available through the Bureau of Vital Records of Missouri.

Registrar and Vital Records hours are Monday through Friday from 8:00 am to 4:00 pm.

The fees for Vital Records are as follows:

Birth Certificate = $15.00 per copy

Death Certificate = $13.00 for the 1st copy and $10.00 for each additional copy for the same person ordered at the same time

You can access an application for a certified copy of a birth or a death certificate below. All applications must be filled out completely, and can be mailed to our office, or presented in person along with a pre-stamped envelope to return. All fees must accompany requests. The health department does not accept out-of-state checks. If applying from out of state, please send money order or cashier's check. Please note, incomplete applications will delay processing.
