Warren County Health Department provides certified copies of Birth and Death Certificates. This service is available through the Bureau of Vital Records of Missouri.

Who can obtain a copy of a vital record?

Available Monday-Friday 8am-4pm

You will need your driver’s license.

The fees for Vital Records are as follows:

Birth Certificate = $15.00 per copy

Death Certificate = $14.00 for the 1st copy and $11.00 for each additional copy for the same person ordered at the same time

You can access an application for a certified copy of a birth or a death certificate below. Completed applications may be presented in person or mailed to our office along with a stamped self addressed envelope for return. All fees must accompany requests. The Warren County Health Department does not accept out of state checks.

Identification Requirements

To help protect against identity theft, identification is required when obtaining a copy of vital record at the Bureau of Vital Records in Jefferson City or at a local agency. A document containing a photo, such as a driver’s license, passport, or school, work or military identification must be presented when applying for a copy of a vital record at the Bureau of Vital Records or a local public health department. If an applicant does not have a photo ID, at least two alternate forms of identification must be presented. [Click here for information on Acceptable ID]

Requests for vital records submitted by mail must be notarized by a notary public.
